• Your activity must receive approval from the BFIS Student Activities Committee before it can begin.

• The form is editable, so you can update it as needed with changes to participants, locations, goals, and more.

• Activities should take place during FLEX period or after school between 4:00-5:15 pm, and in designated campus locations.

• Emergency contacts are provided for your safety and well-being.
